Spinal Cord InjuryCOVID-19Rehabilitation

Simple Explanation

The Spinal Injury Rehabilitation Centre (SIRC) in Nepal experienced a significant COVID-19 outbreak in August 2020 among individuals with spinal cord injuries. The hospital faced challenges due to its open ward setting, crowded facilities, and the need for caregivers to stay with patients, increasing the risk of transmission. Despite implementing various preventive measures, including screening, masking, and hygiene protocols, the virus spread, leading to the conversion of the rehabilitation center into a COVID-19 isolation center.

Key Findings

  • 1
A total of 103 out of 238 individuals tested positive for COVID-19 during the outbreak, including patients, caregivers, staff, and vocational training participants.
  • 2
Strong patient and family input led to the decision to allow one family member to stay with patients, even if COVID-19-positive, highlighting the importance of family support.
  • 3
The experience highlighted the need for more consistent physical distancing, a separate accessible isolation facility, and comprehensive pre-outbreak planning to mitigate the impact of future outbreaks.

Research Summary

In August 2020, the Spinal Injury Rehabilitation Centre (SIRC) in Nepal experienced a large COVID-19 outbreak among individuals with spinal cord injury (SCI). The center is located one hour outside the capital city. Our 74 staff members commute to SIRC using both public transportation and a shared staff bus. We’ve learned critical lessons that will help us and similar hospitals better prepare for the future. Having plans and protocols in place before our first hospitalized individual with COVID-19 was critical.

Practical Implications

Preparedness Planning

Hospitals, especially rehabilitation centers, need comprehensive pre-outbreak plans and protocols to quickly adapt and respond to infectious disease outbreaks.

Infection Control Measures

Consistent physical distancing, hand hygiene, and PPE use are crucial to prevent the spread of infections in high-risk settings like rehabilitation centers.

Accessible Isolation Facilities

Dedicated isolation facilities that are fully accessible for individuals with specific needs, such as those with SCI, are essential to reduce the risk of further spread within healthcare centers.

Study Limitations

  • 1
Limited resources for PPE and isolation facilities.
  • 2
Challenges in maintaining physical distancing due to crowded facilities.
  • 3
Dependence on family caregivers, making complete isolation difficult.
